Medication forms

If your child requires prescription medications at school, such as an inhaler or epi pen, those need to be hand delivered by an adult to the office along with a completed Prescription Medical Form and a completed Permission to Administer Medication form. All non-prescription medications that your child may need during school also must be hand delivered by an adult to the office along with a completed Non-Prescription Medical Form.
